DUI clients only

DUI counterattack treatment required:

1. Make sure a consent form has been signed to come to Family Center of Temple Terrace (FCOTT)

2. If consent is signed, give DUI counterattack 3-5 business days to sent packet over

3. Once received, FCOTT will follow up with the number on the packet

4. An evaluation is scheduled to determine if treatment is required and if so how many sessions (we do not know how many or how long it will take until evaluation has been completed)

5. FCOTT does not do just group sessions, this is supplemental and may be added to individual sessions

DUI counterattack treatment recommended/Probation or Court referral:

1. Records from DUI counterattack must be obtained so client must sign a consent form with FCOTT so records can be requested

2. Once received, FCOTT will follow up with the number on the packet

Cost of Evaluation: $35 Cost of Individuals: $70 Cost of Groups: $30
